ABSTRACT

Resumen: La oxigenación por membrana extracorpórea (ECMO) es de gran utilidad al proveer soporte ventilatorio a pacientes con hipoxia, pero su utilidad en el manejo de pacientes con obstrucción central de la vía aérea y riesgo vital no ha sido frecuentemente usada. La broncoscopía intervencional como terapia bajo ventilación convencional es de alto riesgo en este tipo de pacientes, pero es posible lograr excelentes resultados al ser asociada a ECMO. Comunicamos el caso clínico de 2 pacientes que presentaban disnea en reposo y falla ven1ila1oria aguda ca1as1rófica debido a una obstrucción casi total del lumen traqueal, de causa tumoral. En ambos pacientes en forma urgente se inició soporte circulatorio mediante ECMO VV, mientras se efectuaba la resección tumoral broncoscópica. Luego de terminada la cirugía traqueal, en ambos pacientes se retiró el soporte, siendo decanulados sin eventos y con una buena evolución clínica posterior. Se discute el beneficio del soporte ECMO en este tipo de pacientes.


Abstract: In patients with severe central airway stenosis bronchoscopy-guided intervention therapy under conventional ventilation conveys a high risk. Extracorporeal membrane oxygenation (ECMO) provides very good cardiopulmonary support, but is rarely used in bronchoscopy-guided interventional therapy. We report 2 patients with resting dyspnea due to severe tumor tracheal obstruction and acute pulmonary failure with imminent vital risk. Both patients were cannulated and the ECMO circuit installed on a nearly emergency basis. Tumors were excised, and the patients weaned from cardiopulmonary bypass uneventfully. Subsequent clinical course was satisfactory in both cases. A brief discussion of this condition is included.

ABSTRACT

Los aneurismas del cayado aórtico representan un desafío, ya que el involucramiento de sus grandes ramas exige una técnica quirúrgica compleja. A partir de la aparición del tratamiento endovascular, una alternativa desarrollada en los últimos años para abordar el cayado aórtico consistió en el tratamiento híbrido de esta patología, mediante la derivación quirúrgica de los vasos del cuello y la posterior exclusión del aneurisma con una endoprótesis. Este método híbrido es conocido con el nombre de debranching, y en forma simplificada consiste en realizar, sin circulación extracorpórea, una serie de puentes entre la aorta ascendente y el tronco braquiocefálico, la arteria carótida izquierda y eventualmente la arteria subclavia izquierda, para permitir avanzar una endoprótesis que cubra toda la luz del aneurisma. Se describe la técnica quirúrgica del debranching híbrido tipo I, sin el auxilio de la circulación extracorpórea e implante anterógrado de la endoprótesis, para los aneurismas del cayado aórtico.


Aortic arch aneurysms represent a major challenge as the involvement of the supra-aortic vessels demands a complex surgical technique. Since the advent of endovascular aortic repair, hybrid treatment of aortic arch disease has emerged in recent years. The procedure consists of surgical bypass of the supra-aortic vessels followed by exclusion of the aneurysm with an endograft. This hybrid method is known as debranching and, briefly, consists in performing bypasses between the ascending aorta and the brachiocephalic artery, the left carotid artery and possibly the left subclavian artery without cardiopulmonary bypass, in order to advance an endograft to cover the entire lumen of the aneurysm. The aim of this paper is to describe the surgical technique of type I hybrid debranching without cardiopulmonary bypass and antegrade endograft delivery to treat aortic arch aneurysms.

ABSTRACT BACKGROUND: The pleural drain insertion site after coronary artery bypass graft (CABG) surgery may alter lung function, especially respiratory muscle strength. The main objective of this study was to compare the effectiveness and safety of use of the left lateral intercostal region versus the subxiphoid position for pleural drainage during elective CABG surgery using extracorporeal circulation (ECC). DESIGN AND SETTING: Randomized trial conducted in a tertiary-level hospital in Porto Alegre, Brazil. METHODS: 48 patients were assigned to group 1 (pleural drain in the left lateral intercostal region) or group 2 (pleural drain in the subxiphoid position). Respiratory muscle strength was measured in terms of maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP), in cmH2O, by means of manovacuometry preoperatively, 24 and 72 hours after drain removal and before discharge from hospital. Painand dyspnea scales, presence of infections, pleural effusion and atelectasis, duration of drain use, drainage volumes and surgical reinterventions were also evaluated. RESULTS: After adjustments, there were no significant differences between the groups at the end of the study (before discharge), in predicted percentages either for MIP (delta group 1: -17.21% versus delta group 2: -22.26%; P = 0.09) or for MEP (delta group 1: -9.38% versus delta group 2: -13.13%; P = 0.17). Therewere no differences between the groups in relation to other outcomes. CONCLUSION: There was no difference in maximal respiratory pressures in relation to the pleural drain insertion site among patients who underwent CABG surgery using ECC. ABSTRACT

Fundamentos: A ventilação mecânica (VM) e a circulação extracorpórea (CEC) estão associadas a um declínio da mecânica pulmonar que pode impactar as trocas gasosas. Objetivo: Avaliar o impacto da mecânica pulmonar sobre a duração da VM e trocas gasosas no pós-operatório de cirurgia cardíaca. Métodos: Estudo de coorte realizado com pacientes submetidos a cirurgia cardíaca. Todos os pacientes foram submetidos a avaliação da mecânica pulmonar (complacência estática e resistência das vias aéreas) e gasometria arterial assim que admitidos à unidade de terapia intensiva (UTI) e foram acompanhados até o momento da extubação e, em seguida, até a alta hospitalar. Para correlacionar as variáveis preditoras com o desfecho, foi utilizado o teste de Pearson. Valores de p < 0,05 foram considerados significativos. Resultados: O estudo incluiu 50 pacientes (46 mulheres, 52%), com idade média de 57,5 ± 13,5 anos. A duração da VM foi de 7,7 ± 3,0 horas, a complacência estática foi de 35,5 ± 9,1 cm H2O, a resistência foi de 6,0 ± 2,3 cm H2O, a duração média de estadia na UTI foi de 2,9 ± 1,1 dias e o índice de oxigenação foi de 228,0 ± 33,4 mmHg. Não houve uma correlação significativa entre a duração da VM e a complacência estática (p = 0,73), porém houve uma forte correlação entre a complacência estática e as trocas gasosas (r = 0,8, p < 0,001). Conclusão: A mecânica pulmonar apresenta forte correlação com as trocas gasosas e fraca correlação com a duração da VM no pós-operatório de cirurgia cardíaca


Background: Mechanical ventilation (MV) and extracorporeal circulation (ECC) are associated with a decline in pulmonary mechanics that may affect gas exchange. Objective: To evaluate the impact of pulmonary mechanics on MV duration and gas exchange in the postoperative period of cardiac surgery. Methods: This was a cohort study in patients undergoing cardiac surgery. All patients underwent evaluation of pulmonary mechanics (static compliance and airway resistance) and arterial blood gas analysis upon admission to the intensive care unit (ICU) and were followed up until extubation and hospital discharge. Results: The study included 50 patients (46 women, 52%) with a mean age of 57.5 ± 13.5 years. The MV duration was 7.7 ± 3.0 hours, static compliance was 35.5 ± 9.1 cm H2O, resistance was 6.0 ± 2.3 cm H2O, mean length of ICU stay was 2.9 ± 1.1 days, and oxygenation index was 228.0 ± 33.4 mmHg. No significant correlation was found between MV duration and static compliance (p = 0.73), but a strong correlation was found between static compliance and gas exchange (r = 0.8 and p < 0.001). Conclusion: Pulmonary mechanics have a strong correlation with gas exchange and a weak correlation with MV duration after cardiac surgery

Abstract Objective: To compare the perioperative incidence rates of hemolysis and inflammatory response in patients undergoing coronary artery bypass grafting with the two main types of cardiopulmonary bypass, centrifugal and roller pumps, and establish correlations among hemolytic and inflammatory changes. Methods: This was a prospective, randomized trial of 60 patients assigned to either roller pump (G1, n=30) or centrifugal pump (G2, n=30) bypass. Markers of hemolysis (serum haptoglobin, lactate dehydrogenase [LDH]) and inflammation (interleukin [IL]1ß, IL-6, and TNF-α) were measured and analyzed. Results: There was no significant between-group difference in the variables of interest. In G1, there was a positive association with IL-6 and TNF-α (P<0.01 and P<0.05, respectively). In G2, there was a positive association with LDH in the postoperative period (P<0.5). At 24h post-cardiopulmonary bypass, there were positive associations between LDH and IL-1ß (P<0.05), LDH and TNF-α (P<0.01), haptoglobin and TNF-α (P<0.05), and LDH and TNF-α (P<0.01) in G1, and between LDH and IL-6 (P<0.01), LDH and TNF-α (P<0.01), and LDH and IL-6 (P<0.01) in G2. Conclusion: There were no significant between-group differences in markers of hemolysis or inflammation. IL-6 and TNF-α were positively associated with duration of cardiopulmonary bypass in G1, while LDH was positively associated with duration of cardiopulmonary bypass in G2. The rate of significant associations between markers of hemolysis and inflammation was higher in the roller pump group (G1). Registration number: ReBEC (RBR-92b9dg).

ABSTRACT

There has been a progressive increase in the use of mechanical circulatory support in our country in the last years, mainly in the short-term: as a bridge to cardiac transplant or recovery, which has allowed to rescue patients from a population that concentrates high rates of complications and mortality. This motivated that the Ministry of Health convened a series of experts in the area of heart failure, transplantation and mechanical circulatory support, representatives of different public and private health centers in Chile, with the objective of developing recommendations on the use of short-term mechanical assistance devices, which would serve as a reference for the proper management of these patients. This clinical experts consensus document contains topics related to: definitions and general concepts; indications; contraindications; specific aspects of starting, follow-up and weaning of the devices; process of transfer of patients in circulatory assistance from one center to another and finally criteria of organization, means and competences to be fulfilled by the centers that offer this therapeutic option.

ABSTRACT

OBJETIVO: Apresentar um método prático de ensino e avaliar o grau de eficácia do aprendizado, comparando alunos que passaram pelo curso durante a graduação básica (sétimo semestre) e após 18 meses (décimo semestre), já no internato hospitalar. MÉTODOS: As vísceras conservadas por refrigeração, com coração, estruturas vasculares do mediastino, traqueia e pulmões, foram fixadas em suporte metálico apropriado. Os vasos supra- -aórticos foram ligados, e uma sonda de Foley foi introduzida retrogradamente para a infusão de água, permitindo a apresentação de procedimentos operatórios. Foram aplicados questionários a 40 alunos do sétimo e décimo semestre. As respostas foram submetidas à análise estatística. RESULTADOS: Foram demonstradas técnicas necessárias para o estabelecimento de circulação extracorpórea, confecção de enxerto aorta-coronária com segmento da artéria carótida obtido no mesmo bloco de vísceras, substituição da valva aórtica por prótese valvar sintética, correção de defeito previamente realizado no septo atrial, posicionamento dos vários tipos de eletrodos utilizados para a estimulação elétrica do coração, demonstração de técnicas empregadas para transplante cardíaco, simulação e correção de ferimentos do coração e da aorta. O grau de adequação da ferramenta avaliada foi positivo, com valores entre 55 e 95%, e houve retenção dos ensinamentos após 18 meses. Conclusão: Este método de ensino foi de fácil preparação e baixo custo, permitindo a realização de operações complexas e despertando interesse durante a graduação em Medicina.(AU)


OBJECTIVE: To present this practical teaching method, and assess the degree of learning effectiveness, comparing students who attended the course during basic undergraduation (seventh semester), and after eighteen months (tenth semester), when already in the Hospital internship. METHODS: Thoracic viscera, with heart, vascular structures of the mediastinum, trachea and lungs, preserved by refrigeration, were fixed in a metallic device. The supra-aortic branches were connected, and a Foley catheter was introduced backwards for water infusion, allowing the presentation of surgical procedures. Questionnaires were applied to 40 students of the seventh and tenth semesters; the answers underwent statistical analysis. RESULTS: The following techniques were demonstrated: establishment of cardiopulmonary bypass; coronary artery bypass grafting with carotid artery segment obtained in the same block of viscera; aortic valve replacement with synthetic valve; patch made with biological material in order to correct a defect previously held in the atrial septum; positioning of the various types of electrodes used for electrical stimulation of the heart; demonstration of techniques performed in cardiac transplantation and simulation and correction of both heart and aorta injuries. The degree of adequacy of such tool was positive, with values between 55 and 95%, and retention of what was taught after 18 months. CONCLUSION: This teaching method requires easy preparation and low cost, allowing the performance of complex operations, attracting medical students' interest.(AU)

RESUMO Objetivo: Caracterizar as modificações na concentração sanguínea do lactato e da saturação de oxigênio em pacientes no pós-operatório imediato de cirurgia cardíaca com circulação extracorpórea. Métodos: Foram coletadas amostras de sangue de 35 pacientes, de forma rápida e aleatória, do acesso arterial e das portas proximal e distal de um cateter pulmonar. Resultados: Não foram verificadas diferenças estatisticamente significantes entre saturação de oxigênio no átrio direito (72% ± 0,11%) e na artéria pulmonar (71% ± 0,08%). A concentração sanguínea de lactato no átrio direito foi de 1,7mmol/L ± 0,5mmol/L, enquanto na artéria pulmonar esta concentração foi de 1,6mmol/L ± 0,5mmol/L (p < 0,0005). Conclusão: A diferença entre as concentrações sanguíneas de lactato no átrio direito e na artéria pulmonar pode ser consequência da baixa concentração de lactato no sangue do seio coronário, já que o lactato é um importante substrato para o miocárdio durante este período. A ausência de diferenças entre saturação sanguínea de oxigênio no átrio direito e na artéria pulmonar sugere extração de oxigênio mais baixa pelo miocárdio, em razão do menor consumo de oxigênio.


ABSTRACT Objective: This prospective study aimed to characterize the changes in blood lactate concentration and blood oxygen saturation in patients during the immediate postoperative period of cardiac surgery with extracorporeal circulation. Methods: Blood samples were collected from 35 patients in a rapid and random order from the arterial line and from the proximal and distal port of a pulmonary artery catheter. Results: The results showed no statistically significant differences between the blood oxygen saturation in the right atrium (72% ± 0.11%) and the blood oxygen saturation in the pulmonary artery (71% ± 0.08%). The blood lactate concentration in the right atrium was 1.7mmol/L ± 0.5mmol/L, and the blood lactate concentration in the pulmonary artery was 1.6mmol/L ± 0.5mmol/L (p < 0.0005). Conclusion: The difference between the blood lactate concentration in the right atrium and the blood lactate concentration in the pulmonary artery might be a consequence of the low blood lactate concentration in the blood from the coronary sinus, as it constitutes an important substrate for the myocardium during this period. The lack of differences between the blood oxygen saturation in the right atrium and the percentage of blood oxygen saturation in the pulmonary artery suggests a lower oxygen extraction by the myocardium given a lower oxygen consumption.

Abstract Background: Myocardial revascularization surgery is the best treatment for dyalitic patients with multivessel coronary disease. However, the procedure still has high morbidity and mortality. The use of extracorporeal circulation (ECC) can have a negative impact on the in-hospital outcomes of these patients. Objectives: To evaluate the differences between the techniques with ECC and without ECC during the in-hospital course of dialytic patients who underwent surgical myocardial revascularization. Methods: Unicentric study on 102 consecutive, unselected dialytic patients, who underwent myocardial revascularization surgery in a tertiary university hospital from 2007 to 2014. Results: Sixty-three patients underwent surgery with ECC and 39 without ECC. A high prevalence of cardiovascular risk factors was found in both groups, without statistically significant difference between them. The group "without ECC" had greater number of revascularizations (2.4 vs. 1.7; p <0.0001) and increased need for blood components (77.7% vs. 25.6%; p <0.0001) and inotropic support (82.5% vs 35.8%; p <0.0001). In the postoperative course, the group "without ECC" required less vasoactive drugs, (61.5% vs. 82.5%; p = 0.0340) and shorter time of mechanical ventilation (13.0 hours vs. 36,3 hours, p = 0.0217), had higher extubation rates in the operating room (58.9% vs. 23.8%, p = 0.0006), lower infection rates (7.6% vs. 28.5%; p = 0.0120), and shorter ICU stay (5.2 days vs. 8.1 days; p = 0.0054) as compared with the group with ECC surgery. No difference in mortality was found between the groups. Conclusion: Myocardial revascularization with ECC in patients on dialysis resulted in higher morbidity in the perioperative period in comparison with the procedure without ECC, with no difference in mortality though.


Resumo Fundamento: A revascularização cirúrgica do miocárdio é o melhor tratamento para o paciente dialítico com doença coronariana multiarterial. Contudo, o procedimento ainda apresenta elevada morbimortalidade. O uso da circulação extracorpórea (CEC) pode impactar de maneira negativa na evolução intra-hospitalar desses pacientes. Objetivos: Avaliar a diferença entre as técnicas com ou sem CEC na evolução intra-hospitalar de pacientes dialíticos submetidos a cirurgia de revascularização do miocárdio. Métodos: Estudo unicêntrico de 102 pacientes dialíticos consecutivos e não selecionados, submetidos à revascularização cirúrgica do miocárdio em um hospital terciário universitário no período de 2007 a 2014. Resultados: 63 pacientes foram operados com CEC e 39 sem o uso de CEC. Foi observada alta prevalência de fatores de risco cardiovascular em ambos grupos, porém sem diferença estatisticamente significante entre eles. O grupo "com CEC" apresentou maior número de coronárias revascularizadas (2,4 vs 1,7; p <0,0001), maior necessidade de hemocomponentes (77,7% vs 25,6%; p <0,0001) e apoio inotrópico (82,5% vs 35,8%; p <0,0001). Na evolução pós-operatória, o grupo "sem CEC" apresentou menor necessidade de drogas vasoativas (61,5% vs 82,5%; p = 0,0340), maior taxa de extubação em sala cirúrgica (58,9% vs 23,8%, p = 0,0006), menor tempo de ventilação mecânica (13,0 horas vs 36,3 horas, p = 0,0217), menor taxa de infecções (7,6% vs 28,5%; p = 0,0120) e menor tempo de internação em UTI (5,2 dias vs 8,1 dias; p = 0,0054) em comparação ao grupo "com CEC". Não houve diferença de mortalidade entre os grupos. Conclusão: O uso da CEC na revascularização do miocárdio em pacientes dialíticos resultou em maior morbidade no período perioperatório em comparação ao procedimento realizado sem CEC, contudo, sem diferença de mortalidade.

ABSTRACT

O tratamento da síndrome coronariana aguda obteve expressiva redução da mortalidade graças ao desenvolvimento de novos fármacos e da angioplastia coronariana primária. Entretanto, apesar desta terapêutica ser eficiente e oferecer resultados adequados no tratamento do infarto agudo do miocárdio (IAM), o choque cardiogênico ainda ocorre em 5 a 10% dos casos e continua a ser a principal causa de mortalidade intra-hospitalar. Portanto, temos um desfecho clínico insatisfatório neste grupo de pacientes com prognóstico reservado a curto e longo prazo. O balão intra-aórtico, embora empregado de forma rotineira no tratamento do choque cardiogênico refratário, apresenta benefícios limitados e não apresenta impacto no prognóstico destes pacientes, como demonstrado em ensaios randomizados recentes. Os sistemas de suporte de vida extracorpóreo (ECLS) são considerados dispositivos eficazes para o tratamento do choque cardiogênico refratário no IAM, uma vez que oferecem fluxo sanguíneo adequado e são instalados rapidamente pela canulação de vasos periféricos. The treatment of acute coronary syndrome has achieved a significant reduction in mortality, thanks to the development of new drugs and primary coronary angioplasty. However, although this therapy is efficient and offers adequate results in the treatment of acute myocardial infarction (AMI), cardiogenic shock still occurs in 5% to 10% of cases, and continues to be the main cause of intra hospital mortality. Therefore, we have an unsatisfactory clinical outcome in this group of patients, with a reserved prognosis in the short and long terms.The intra-aortic balloon pump, although used routinely in the treatment of refractory cardiogenic shock, has limited benefits and does not have any impact on patient prognosis, as demonstrated in recent randomized trials. Extracorporeal life support systems (ECLS) are considered effective devices for the treatment of refractory cardiogenic shock in AMI, as they offer adequate blood flow and are rapidly installed through cannulation of the peripheral veins. They therefore enable medical decision-making for diagnostic and therapeutic interventions – decision-making bridge – for treatment of the clinical and anatomical cause, with consistent and encouraging clinical results for the use of this type of technology...

ABSTRACT

Os avanços nas técnicas cirúrgicas e manejo clínico aumentaram a sobrevida das crianças com cardiopatia congênita. Eventos tromboembólicos representam importante causa de morbimortalidade nestes pacientes, principalmente no pós-operatório da cirurgia de Fontan e implante de próteses, acrescido do risco adicional do seu tratamento. Particularidades próprias da idade, fatores hemodinâmicos e limitações na investigação de trombose contribuem para a dificuldadede definir diretrizes para anticoagulação em crianças. Os medicamentos mais usados na fase aguda são: heparina não fracionada e de baixo peso molecular, enquanto em longo prazo os antagonistas da vitamina K, principalmente a varfarina. O manejo seguro de anticoagulação nesta faixa etária é dificultado pela estreita faixa terapêutica, interação alimentar, uso simultâneo de vários medicamentos, além da dificuldade de punção venosa frequente e, com frequência os níveis desejáveis não são atingidos. Ainda faltam dados que deem suporte para recomendação de anticoagulação em crianças. Os novos anticoagulantes como os inibidores diretos da trombina e os inibidores seletivos do fator Xa, são empregados em adultos, principalmente com fibrilação atrial, o uso pediátrico ainda não está definido. Esta revisão sumariza as indicações atuais do uso de anticoagulantes e antiagregantes plaquetários em crianças com cardiopatia congênita.


Advances in surgical techniques and clinical management have led to increased survival ofchildren with congenital heart disease. Thromboembolic events represent a major cause of morbidity and mortality in these patients, particularly following Fontan surgery and the implantation of prosthesis, added to the risk of their treatment. Characteristics such as age, hemodynamic factors and limitations in the investigation of thrombosis have contributed to the difficulty of defining guidelines for anticoagulation in children. The most commonly used medications in the acute phase are and low molecular weight heparin, and in the long term, vitamin K antagonists, particularly warfarin. The safe management of anticoagulation in this age range is hindered by the narrow therapeutic range, food interactions, simultaneous use of other medications, and the difficulty of frequent venous puncture. As a result, the desired levels are often not reached. There is also a lack of data to support the recommendation of anticoagulation in children. The pediatric use of the new anticoagulants, such as direct thrombin inhibitors and selective factor Xa inhibitors used in adults, particularly with atrial fibrillation, is not yet defined. This review summarizes the current indications of the use of anticoagulants and platelet antiaggregants in children with congenital heart disease.

ABSTRACT

A cardiopatia reumática é um importante problema de saúde pública. Há escassez de dados sobreinfluência de variáveis cirúrgicas na mortalidade de pacientes reumáticos submetidos dupla-troca valvar (DTV).Objetivo: Identificar possíveis variáveis cirúrgicas associadas à mortalidade de pacientes reumáticos submetidos à DTV.Métodos: Estudo retrospectivo de corte transversal, incluindo 104 pacientes >18 anos, com diagnóstico prévio devalvopatia reumática, submetidos à cirurgia de DTV no período de janeiro de 2007 a dezembro de 2011. A coletade dados utilizou os prontuários de pacientes do Hospital Ana Nery, Salvador, BA, Brasil.Resultados: Observou-se diferença estatística significativa entre os grupos do desfecho (óbito intra-hospitalar/alta hospitalar) em relação às variáveis, respectivamente: tempo de anoxia (minutos) de 149,17±40,99 e 123,99±24,12(p=0,001); tempo de CEC 185,53±54,59 e 157,34±34,62 (p=0,006); e o tempo cirúrgico total 350,29±56,69 e 295,23±63,98(p=0,002). Os pacientes que realizaram outro procedimento associado à DTV, no mesmo tempo cirúrgico,apresentaram maior mortalidade (n=10; 31,2%), em relação aos que realizaram apenas a DTV (n=9; 12,8%) (p=0,027).Houve também associação significativa na comparação de pacientes com reabordagem cirúrgica com aqueles querealizaram única abordagem (p<0,001). Não houve diferença estatística quando se comparou desfecho hospitalare os tipos de próteses utilizadas (p=0,219). Conclusões: As variáveis cirúrgicas que tiveram influência na mortalidade foram: tempos de anoxia, de CEC, de cirurgia total, com possíveis pontos de corte, respectivamente, de 150 min, 100 min e 300 min. Rheumatic heart disease is a major public health issue. Data on the influence of surgical variables in mortality in rheumatic patients undergoing double valve replacement (DVR) are scarce. Objective: Identify potential surgical variables associated with mortality in rheumatic patients undergoing DVR. Methods: Retrospective cross-sectional study including 104 patients over 18 years, previously diagnosed with rheumatic heart-valve disease, and undergoing DVR surgery from January 2007 to December 2011. Data collection: medical records of patients from HospitalAna Nery, Salvador, Brazil.Results: Outcome groups (in-hospital death vs. hospital discharge) had a significant statistical difference in relation to variables, respectively:anoxia time (in minutes) of 149.17±40.99 and 123.99±24.12 (p=0.001); CPB time 185.53±54.59 and 157.34±34.62 (p=0.006); and totalsurgical time 350.29±56.69 and 295.23±63.98 (p=0,002). Patients who underwent another procedure associated with DVR for the same surgical time showed higher mortality rates (n=10; 31.2%) compared to those who underwent DVR only (n=9, 12.8%) (p=0.027). There was also a significant association when comparing patients with surgical rapprochement with those who underwent one surgery only(p<0.001). There was no statistical difference between hospital outcome and the types of prostheses used (p=0.219). ABSTRACT

PURPOSE: To evaluate a new perfusate solution to be used for ex vivo lung perfusion. METHODS: Randomized experimental study using lungs from rejected brain-dead donors harvested and submitted to 1 hour of ex vivo lung perfusion (EVLP) using mainstream solution or the alternative. RESULTS: From 16 lungs blocs tested, we found no difference on weight after EVLP: Steen group (SG) = 1,097±526g; Alternative Perfusion Solution (APS) = 743±248g, p=0.163. Edema formation, assessed by Wet/dry weigh ratio, was statistically higher on the Alternative Perfusion Solution group (APS = 3.63 ± 1.26; SG = 2.06 ± 0.28; p = 0.009). No difference on PaO2 after EVLP (SG = 498±37.53mmHg; APS = 521±55.43mmHg, p=0.348, nor on histological analyses: pulmonary injury score: SG = 4.38±1.51; APS = 4.50±1.77, p=0.881; apoptotic cells count after perfusion: SG = 2.4 ± 2.0 cells/mm2; APS = 4.8 ± 6.9 cells/mm2; p = 0.361). CONCLUSION: The ex vivo lung perfusion using the alternative perfusion solution showed no functional or histological differences, except for a higher edema formation, from the EVLP using Steen Solution(r) on lungs from rejected brain-dead donors. .

ABSTRACT

Surgery of the ascending aorta and its arc with extracorporeal circulation require an arterial cannulation that assures and adequate systemic circulation and reduces the risk of neurological complications. A peripheral cannulation such as the femoral artery, a central extra-thoracic approach such as axillary, subclavian or carotid arteries or a central intra-thoracic approach such as aorta and innominate arteries, are valid alternatives. Innominate artery cannulation can be carried out without the need of a second incision. It is an easily accessible and large bore vessel. It allows an anterograde brain and visceral flow. In cases of aortic arch surgery, the clamping of the artery in its origin, allows a selective anterograde brain perfusion. The surgical technique for innominate artery cannulation is herein described.


La cirugía de la aorta ascendente y cayado con circulación extra corpórea requiere de una canulación arterial que permita asegurar una perfusión sistémica adecuada y disminuir las complicaciones neurológicas. El sitio ideal de canulación es aún un tema de discusión entre los cirujanos. Entre las alternativas de abordaje están la canulación periférica (arteria femoral), la canulación central extratorácica (arterias axilar, subclavia o carótidas) y la canulación central intratorácica (aorta, arteria innominada). La canulación de la arteria innominada es una técnica que se puede realizar sin necesidad de una segunda incisión, es un vaso de fácil abordaje y habitualmente de gran calibre. Permite entregar un flujo cerebral y visceral anterógrado y, en caso de cirugía sobre el cayado, el pinzamiento de la arteria a nivel de su origen permite entregar una perfusión cerebral selectiva anterógrada. Se describen la técnica quirúrgica y los reparos operatorios que nos parecen importantes en su realización.

INTRODUÇÃO: A circulação extracorpórea (CEC) é indispensável para a maioria das operações cardíacas, mas causa danos significantes ao sangue, dentre eles a hemólise. OBJETIVO: Quantificar as taxas de hemólise em diferentes tempos nas operações para revascularização do miocárdio com uso de CEC. MÉTODOS: Foram medidas as taxas de hemólise de 22 pacientes em 6 tempos distintos durante a revascularização do miocárdio com uso de CEC: T0 - antes do início da CEC, T1 - 5 minutos após o início da CEC, T2 - com 30 minutos de CEC, T3 - imediatamente antes do despinçamento da aorta, T4 - imediatamente antes da passagem do volume residual para o paciente e T5 - 5 minutos após o término da passagem do volume residual para o paciente. Foram calculadas as taxas de hemólise entre os intervalos de tempo: T0-T1; T1-T2; T2-T3; T3-T4 e T4-T5. RESULTADOS: Os primeiros 5 minutos após a CEC demonstraram maior taxa de hemólise (P = 0,0003) em comparação às outras taxas calculadas, representando 29% da hemólise total até T4 (imediatamente antes da passagem do volume residual para o paciente). CONCLUSÃO: Não foram observadas variações significantes nas taxas de hemólise durante a aspiração na raiz da aorta (P > 0,38) nem com o procedimento utilizado para a passagem do volume residual de sangue no circuito para os pacientes.


INTRODUCTION: Extracorporeal circulation (EC) is very important in cardiac surgery but causes significant damage to the blood, including hemolysis. OBJECTIVE: To quantify the rate of hemolysis at different times during EC in elective coronary artery bypass grafting. METHODS: We measured rates of hemolysis of 22 patients at 6 different times during myocardial revascularization during EC: T0 - before the start of EC, T1 - five minutes after of the EC initiation, T2 - 30 minutes of EC, T3 - immediately before the aortic unclamping, T4 - immediately before passage of the residual volume to the patient and T5 - five minutes after the passage of the residual volume to the patient. Rates of hemolysis were calculated between the intervals of time: T0-T1; T1-T2; T2-T3; T3-T4 and T4-T5. RESULTS: The first 5 minutes after the EC showed the highest rate of hemolysis (P = 0.0003) compared to the others calculated rates, representing 29% of the total haemolysis until T4 (Immediately before passage of the residual volume to the patient). CONCLUSION: There were no significant changes in the rate of hemolysis during the suction in the aortic root (P> 0.38), nor with the procedure used for the passage of the residual volume of blood in the circuit to the patient.

ABSTRACT

INTRODUÇÃO: A cirurgia de revascularização do miocárdio (RM) sem circulação extracorpórea (CEC) é uma técnica amplamente utilizada. A fluxometria coronariana é a técnica mais usada para avaliação dos enxertos, porém, poucos estudos comparam os dados fluxométricos na RM com e sem CEC. O objetivo deste estudo foi comparar as variáveis fluxométricas dos enxertos de artéria torácica interna esquerda para a artéria descendente anterior em pacientes submetidos à RM com e sem CEC. MÉTODOS: Entre março e setembro de 2010, foram analisados retrospectivamente 35 pacientes consecutivos, não randomizados, submetidos à RM. Foram alocados 10 pacientes no grupo A (com CEC) e 25 no grupo B (sem CEC). O fluxo médio do enxerto (FME), o índice pulsátil (PI) e a porcentagem de enchimento diastólico (ED) foram obtidos por meio da fluxometria por tempo de trânsito. Foi utilizado o teste exato de Fisher e Mann-Whitney, sendo considerado estatisticamente significante P<0,05. RESULTADOS: Não houve óbito, infarto agudo do miocárdio ou necessidade de angioplastia em 30 dias de pós-operatório. O número médio de anastomoses distais foi 2,3 ± 0,8 por paciente no grupo sem CEC, e de 2,2 ± 0,6 no grupo com CEC (P=0,10). A mediana do fluxo médio do enxerto foi 23 ml/min, no grupo A, e 25 ml/min, no grupo B (P=0,34). A percentagem de enchimento diastólico foi 56%, no grupo A, e 59,5%, no grupo B (P=0,86). O índice pulsátil foi 2,3, no grupo A, e 2,2, no grupo B (P=0,82). CONCLUSÃO: Não houve diferença nos parâmetros fluxométricos (FME, ED e PI) encontrados nos pacientes submetidos à revascularização do miocárdio com e sem CEC.


BACKGROUND: Off-pump coronary bypass grafting (OPCAB) has become a widely used technique. Coronary flowmetry is the most common method employed to assess graft patency, nevertheless, few studies compare flow patterns between ONCAB and OPCAB surgery. The objective of this study was to compare flowmetry data in left internal mammary artery grafts bypasses to the left anterior descendent artery. METHODS: From March to September of 2010, thirtyfive consecutive, non-randomized patients underwent CABG and were retrospectively evaluated. Ten patients were located on group A (On Pump), and twenty-five on group B (Off Pump). The mean graft flow (MGF), pulsatile index (PI) and diastolic filling (DF) were obtained using Transit Time Flowmetry (TTFM). The Fisher exact test, and Mann Whitney test were used, and a P value of < 0.05 was considered to indicate statistical significance. RESULTS: There were no deaths, AMI, re-interventions or PTCA in a 30-day period. The number of bypasses performed per patient was 2.3 ± 0.8 in the OPCAB group, and 2.2 ± 0.6 in the ONCAB group, with no significant difference (P=0.10). The median of mean flow was 23 ml/min on group A, and 25 ml/min on group B (P=0.34). Diastolic filling percentage was 56% on group A, and 56.9% on group B (P=0.86). Pulsatile Index was 2.3 on group A, and 2.2 on group B (P=0.82). CONCLUSIONS: There was no difference between TTFM values (MF, PI and DF) in patients operated with or without cardiopulmonary bypass.

ABSTRACT

Introducción: la circulación extracorpórea normotérmica en pediatría fue iniciada por Lecompte en 1995. Los reportes europeos son grandes series de casos y pocos estudios controlados. Objetivo: evaluar el empleo de la normotermia como método de protección de órganos durante la circulación extracorpórea en niños, para intentar obtener un mayor nivel de evidencia médica e introducirla en todo el universo de trabajo. Métodos: se realizó un estudio explicativo, controlado y prospectivo, en 100 pacientes con edades comprendidas entre los 30 días y los 18 años, que se distribuyeron aleatoriamente, con el objetivo de evaluar la normotermia durante la circulación extracorpórea. En normotermia (n= 45) se utilizó temperatura de 36º C, hematócrito mayor de 30 por ciento y flujo de perfusión de 2,8 a 3,5 L/min/m². En hipotermia (n= 55), hematócrito de 25 a 30 por ciento y flujo de perfusión de 2,2 a 2,8 L/min/m². Resultados: ambos grupos fueron homogéneos en edad, peso y riesgo ajustado para la cirugía cardiaca. Los índices de inotrópicos y de sangrado, la frecuencia del bajo gasto cardíaco, los tiempos de circulación extracorpórea, la ventilación mecánica y de estadía en la unidad de cuidados intensivos, fueron significativamente menores con la normotermia. El riesgo relativo de bajo gasto cardiaco fue 2,62 con el uso de la hipotermia. La reducción absoluta del riesgo para dicha complicación, empleando la normotermia, fue 18 por ciento. Conclusiones: la normotermia durante la circulación extracorpórea en pediatría es tan segura y más eficaz que la hipotermia. La derivación cardiopulmonar normotérmica es perfectamente factible, y la necesidad de convertirla a hipotérmica es muy poco frecuente(AU)


Introduction: the normothermia extracorporeal circulation in pediatrics was started by Lecompte in 1995. The European reports are large series of cases and few controlled studies. Objective: to assess the use of the normothermia as a protecting method of the organs during the extracorporeal circulation in children, trying to obtain a higher level of medical evidence to its introduction in all the work universe. Methods: a prospective, controlled and explanatory study was conducted in 100 patients aged between 30 days and 18 years randomized distributed to assess the normothermia during the extracorporeal circulation. In the normothermia (n= 45) we used a 36º C temperature, hematocrit higher than 30 percent and a (2.8 to 3.5) L/min/m² perfusion flow. In hypothermia (n= 55), hematocrit from 25 to 30 percent and perfusion flow of (2.2 to 2.8) L/min/m². Results: both groups were homogenous in age, weight and risk fitted for cardiac surgery. The inotropic rates and of bleeding, frequency of the low cardiac output, times of the extracorporeal circulation, mechanical ventilation and os the stay in the intensive care unit (ICU) were significantly lower with normothermia. The relative risk of low cardiac output was of (2.62) with the use of hypothermia. The absolute reduction of risk for such complication, using normothermia was of 18 percent. Conclusions: the normothermia during extracorporeal circulation in pediatrics is more sage and effective than the hypothermia. The normothermia cardiopulmonary shunt is perfectly feasible and the need of to convert in hypothermia is not frequent(AU)
